Arkham Island was a perfect location for construction for large projects after the meteor shower. While the family home was in shambles, it was still salvageable but there was still a great deal more land that could be put to good use. The Arkham family reached out to The Crowne Family for funding for their project called Arkham Hospital. Gotham was still recovering from the meteor storm and the city desperately needed a place to send the homeless or mentally unstable. Given the funding and support they needed, construction began in 1905. While construction should have created a ray of hope for the people in need for Gotham, it did not. Instead, it begins with the architect becoming mentally unstable and using an axe to slaughter his workers. Detained for the rest of the construction, he was the first person to spend the rest of his life in the asylum. Construction was completed in 1918 and was renamed Elizabeth Arkham Asylum for the Criminally Insane after Amadeus Arkham's mother to remember her after she committed suicide during the final stages of construction. However, it is believed by many that Amadeus had actually euthanized his mother but there has been no investigation to prove one way or another.

According to Amadeus, he was determined to prevent anyone from facing the same fate as his mother and from day one took pride in properly treating the mentally ill. Despite his best intentions, the asylum's quality of care gradually began to decline. Determined to more closely monitor what was being done in the asylum, Amadeus moved his wife Constance, and daughter Harriet into the family estate to keep him closer to the asylum. However the move lead to personal tragedy. One of the more violent patients escaped the asylum and broke into the hope, killing his wife and daughter while Amadeus was in the city for the day. Upon arriving home, Amadeus was greeted by the sight of the corpses of his family. Emotionally destroyed, Amadeus' mental state began to decline as he begins to design a floor plan that would revolve around occult runes that he believed would drive away the monsters haunting his dreams.

Over the next several years, inmates were dying during the sessions conducted by Amadeus Arkham. For some time, the staff treated the deaths like accidents but it became more evident that Amadeus was gradually descending into mental illness. Eventually Amadeus Arkham was admitted as a patient in his own asylum, where he carves messages into the walls and belting out the Star-Spangled Banner.

Amadeus never recovers from his mental illness and dies in the asylum while his nephew, Jeremiah, takes over as the head of Arkham Asylum. During his time as the head of Arkham Asylum, Jeremiah saw that there were changes that needed to be made. Not only did he believe that the treatments were unregulated and out of control, but the building was in dire need of renovation. It's thanks to the Mental Health Systems Act of 1980 and Thomas Wayne that Jeremiah is able to get the funding he needs to do the renovations. Creating a more sanitary and safe structure, Jeremiah changes the name to Arkham Island Mental Health Facility. With the hopes that these changes will give Arkham a second chance to have a bright future. Even though the name was changed before the start of the renovations in 1987, the general population continued to refer to Arkham as Arkham Asylum. To Jeremiah's frustration, the repeal of the Mental Health Systems Act forced him to cut back on the majority of the renovations. Now forced to be more selective about what is renovated and what is left as is, Arkham is destined to have most of the original construction with only moderate changes to the structure itself. Even after completion in 1994, the facility could not escape its past and people continue to call it Arkham Asylum. Determined to try to turn things around Jeremiah continued to do everything in his power to improve the quality of the care, despite the last of the funding drying up once Thomas Wayne passes away.
